RELEASE FORM

           For and in consideration of my being allowed to participate in the Harmony Highlands Singing School, I and my parents agree on behalf of myself, my parent(s), guardian(s) heirs, executors, administrators and assigns, to hold harmless, release, acquit, indemnify and forever discharge the sponsors, and all other persons who are or may be liable, of any and all actions, claims, demands, costs, loss of services, liabilities, loss, damages, or expenses of whatever kind, compensation, or for any other consequential damage on account of or in any way arising out of my participation in the Harmony Highlands Singing School, which may at any time be incurred by reason of my participation in or preparation for any singing school activity.

           In the event of an emergency, I and my parents do hereby authorize the obtaining of medical treatment for myself/my child(ren) at my expense.  Furthermore, I do hereby acknowledge my responsibility to disclose any information regarding medications, prescriptions or medical conditions.  By signing this Release, I am affirming that I have read the purpose, guidelines, and regulations of Harmony Highlands Singing School, that I understand them, and that I agree to abide by such purpose, guidelines and regulations.
